To submit an incident report to the Greek Council Office and the Assistant Dean of Students, please complete the information below to the best of your ability.  Once your report is received, the Greek Council Office and/or the Dean of Students Office will begin investigating the reported incident.  If there is an emergency that must be addressed, please contact the Greek Council On-Call Phone at 801-232-2363 or the Salt Lake City Police Department at 801-799-3000.
